Complete this form confirm your child is receiving the proper in-class driver’s education as part of the requirements when applying for a MN driver’s license.

Drivers with inadequate eyesight must submit this form as part of a vision exam that is required in order to apply for a driver’s license in MN.
Minnesota Change of Address Forms
If you are moving within the state of Minnesota, you are required to report your new address to the DVS on time.
Address change on MN driver’s license
Once you moved to a new address in MN, you must change your address on your driver’s license or ID card within 30 days of moving. You can only update your address either by replacing or renewing your driver’s license, and paying the appropriate fees.
For driver’s license or ID cards replacements, here’s a guide of accepted proof of identity documents to complete this procedure.
To renew your driver’s license or ID cards, visit your local DVS in person as this procedure cannot be handled online or by mail.
Here’s a checklist of steps you need to follow when renewing your driver’s license with your new address in person:
- Fill in an application form
- Take a vision exam
- Take a picture
- Provide your MN driver’s license or ID card
- Give your Social Security Number
- Pay the driver’s license renewal fee
- Minors must have their parents/ legal guardians sign their applications, if they are under 18 years old.
Address change on MN vehicle title and registration
You are allowed to update your address on your vehicle title and registration online or in person at any local MN DVS agency.
