Submit this form if you are required to take an eye exam prior to getting a driver’s license. This form must be filled out by a vision specialist.

Intermediate Permit Driving Exception – Form FSGDL-02 | Download
Complete this form to request permission to practice driving a vehicle out of the permitted times in case you are a minor driver. This form must be signed by a parent or legal guardian.

Complete this form to update your address information with the WY DOT.

Note that you are allowed to use this form to change your address information on your driver’s license and it should updated within 10 days of moving. This could be done either by fax, by mail, or in person.
